The Department of Health proposes a midpoint review of The National Service Framework for Long Term Conditions, and invites those interested to take part in an online survey to help develop the review.

The survey asks, for example, if the review should cover service provision and patient experience, and who should take part.

Make sure you have your say. The survey consists of 6 very straightforward questions.
